To save the form to your hard drive you must have Adobe Acrobat installed:
Open the form.  Select print.  From the printer dialog box select, "print to pdf".
This will allow you to save the file to a location on your hard drive

To submit your application online after completion,

  • Go to your email server
  • Compose a message
  • Attached the completed application to the email
  • Send it to chammond@southark.edu


Please note, recommendation forms must be hand written only.
